You almost always replace camshafts, right?

If you have it out, why not. Not always necessary but it's a fairly inexpensive upgrade that gives you a ton of more power for the buck. Of course, change the cam bearings and look over your lifters real good.
I generally go with what they used to call an "RV Cam". Stock lift with longer duration. Gives you much increased power in the 2500, 3000 to whatever range.

Replace, I'm going with a slightly more aggressive version of the 068 cam found in early Ram Air Trans Ams (it will increase torque quite a bit while still remaining streetable and create low enough compression to run 87 octane).
